S.A.F.E.

Sexual Assault Forensic Exam

When a child has possibly experienced a sexual assault, a S.A.F.E will be requested.  A S.A.F.E. is a comprehensive, non-invasive medical examination that is much like a well child check-up.  We do not restrain children during exams and it is the child’s choice whether the exam is completed.  Children may choose to have a support person present.  During exams, we offer many stress reducing activities for children such as playing "I spy”, blowing bubbles, and looking at books or magazines. Also, our medical staff reinforces positive self-image with children following their exam.

Who needs a SAFE?

Any allegation of penetration (including oral, anal, etc.).

A young or nonverbal child that is unable to give a thorough disclosure.

Siblings of a child who has a positive exam or who gave a verbal disclosure regarding abuse.

Any child who has symptoms of an STD (discharge, burning with urination, abdominal pain, sores or a rash in the genital area or throat, itching of the genitals, or a sore throat following oral-genital contact).

Who will perform the exam?

A nurse practitioner or physician performs the exam. They have received specialized training through Missouri's Sexual Assault Forensic Exam/Child Abuse Resource and Education (SAFE-CARE) Network to recognize the signs of abuse and make the exam process as easy as possible for the child. The SAFE-CARE Network requires that examiners receive an initial two-hour training to become certified.  Examiners are required to attend a one-hour training in child abuse investigation annually in order to maintain their certification.  Our medical assistant prepares children for the exam process by explaining each step of the exam in detail and is present to offer support to the child and their caregiver. 

How long will the exam take?

Appointments at the Center last an average of one to two hours depending upon the services to be offered.  Before the exam, the caregiver and SAFE provider will discuss the child's medical and social history. The team may use this time to conduct the forensic interview with the child, if one has been requested. The medical assistant may also use this time to begin preparing the child for the exam. The actual exam lasts about 20 minutes.  If the child must spend any time waiting for an examination or interview, a staff member or volunteer engages the child in an activity such as coloring, board games, reading or watching a video.

Preparing a child for a visit

Parents are encouraged to prepare their child for his/her visit to the Center by explaining that this is a place that children come to talk to someone about what may have happened to them. Parents are instructed to tell their child to tell the truth and that it is “ok” for him/her to talk to our interviewer.  In addition, the staff strongly encourages parents to tell children that they will be told that they will have a head-to-toe check up (if one is scheduled) and that a medical provider will look at all of their parts.  Parents are given age appropriate explanations to share with their children regarding the exam.

How much does the exam cost?

Families are NEVER billed for exams or interviews.

We seek reimbursement through Medicaid, private insurance, Children's Division (Department of Family Services), law enforcement, private insurance, or through the Department of Health.
